FOR HEALTHCARE PROVIDERS

Supporting your patients
in the work that happens

between visits.

MedMentor provides structured education and implementation support for patients with cardiometabolic findings — working alongside physicians, never in place of them.

Dr. Tony DeFrance, MD

Triple Board-Certified: Interventional Cardiologist · Cardiac CT Angiography Specialist · Lifestyle Medicine Physician (ABLM) · IFM Cardiometabolic Certified

“I built MedMentor because I kept seeing the same gap — patients leaving clinical encounters with important findings and nowhere to go for structured, evidence-based implementation support. This isn’t coaching. It’s a clinical education platform led by a physician, designed to reinforce exactly what you’ve already told your patient.”

CLINICAL BOUNDARIES

Clear Scope.  Clear Limits.

MedMentor does not establish a physician-patient relationship and does not communicate clinical findings back to referring providers. Patients manage their own ongoing clinical care relationship independently.

Which Patients Benefit Most?

MedMentor is well-suited for patients who have received a cardiometabolic finding and need structured support translating clinical recommendations into sustained action — particularly those where lifestyle intervention is the primary next step.

CCTA showing non-obstructive plaque or soft plaque in the coronary arteries

Metabolic syndrome, insulin resistance, or prediabetes with lifestyle modification as first-line

Elevated cardiovascular risk with motivation but limited time or structure to act independently

Patients who want to understand their findings more deeply than a clinical visit allows

Not Appropriate For

Acute cardiac conditions requiring urgent management

Significant stenosis or obstructive coronary disease requiring intervention

Patients requiring medication titration or complex clinical management

Second opinions on clinical treatment decisions
